Wanted Looking for Cox & Yeman Nameplate

I'm looking for a nameplate for my Cox & Yeman table. I believe it will look something like this:



If anyone has a spare one that they are willing to let me have for a reasonable price, please let me know. I'd need to know the size to check that it will fit in my rebate.
