Without seeing your action its not possible to give a cast iron answer but i will give you some pointers.
First of all it would seem you have some lateral movement in your cueing. This can be caused by dropping the elbow excessively on the shot. It can also be linked to too tight a grip at the backswing, again causing to lift the cue therefor dropping the butt end on follow through.
What you need to remember is, the follow through and backswing are mirror images of each other.
Meaning if you are lifting the tip through on the follow through, it is more than likely you are lifting the cue up in the backswing, this would also apply to any sideway movement.
A couple of good training drills is to use the neck of a plastic drinks bottle. and train yourself to put the tip back and froward through the bottle without making contact, or cueing along the baulk line and along the cushion rail.
Both will develop muscle memory and will take several sessions to remedy your fault.
