Without seeing your action its not possible to give a cast iron answer but i will give you some pointers.
First of all it would seem you have some lateral movement in your cueing. This can be caused by dropping the elbow excessively on the shot. It can also be linked to too tight a grip at the backswing, again causing to lift the cue therefor dropping the butt end on follow through.
What you need to remember is, the follow through and backswing are mirror images of each other.
Meaning if you are lifting the tip through on the follow through, it is more than likely you are lifting the cue up in the backswing, this would also apply to any sideway movement.
A couple of good training drills is to use the neck of a plastic drinks bottle. and train yourself to put the tip back and froward through the bottle without making contact, or cueing along the baulk line and along the cushion rail.
Both will develop muscle memory and will take several sessions to remedy your fault.
I hope this helps. Yours The Doctor.

Remember that if your head stays perfectly still throughout the delivery of the cue AND it stays touching the cue as it follows through then the cue cannot possibly hit A LOT higher than intended. That is my opinion.

One possibility is that you might be gripping your cue further back from the vertical. What happens then is that your cue tends to 'see saw' on the follow through with your cue tip elavated slightly during the final phase of your stroke.

Also, your cue might be too low, ie, at adress, the shaft of your cue is just millimeters above the rail. What happens is that when following through, your shaft gets thicker as you push the cue forward and this results in the shaft contacting the rail and pushing your tip upwards. A remedy for this is to have slightly more 'breathing room' between shaft and rail at address.

Could be lots of things and would agree with most of the ones mentioned. I think you need to get someone to video you at your address position so that you can see how high up the white you are aiming. Some people will swear to aiming at the bottom or top when in reality they are no-where near. You then need to move onto actually striking the white where you are aiming. If you are not doing this ie you are aiming at one place then striking another then the problems listed above are good starting points. I think videoing yourself would definately help and you may even need to see a coach to solve the problem. I would tend to think your grip is too tight or you are dropping your elbow too much on the delivery of the cue but this is only guesswork from what you have said.

I hope you solve the problem. Let us know.
