It does strike me as odd that nobody has suggested that this fault could actually be a consequence of contorting the body into position having approached the shot out of alignment in the 1st place. Could it not be that the grip is fine and naturally finishing the delivery tries to pull/twist the cue into natural alignment?
It's like trying to cut a straight line with a circular saw whilst standing slightly off line. You can do it slowly and deliberately, but make an attempt to go faster and the body's natural movement will move the cut off line easily.
I'd revisit the basics of getting down to the shot in a dead straight aligned movement before messing with the grip/butt size etc. Just my 2p
