    There must be some pre history. I don´t think Chris Small fell of the tour and wasn´t someone else called in on the tour, when Paul Hunter had his place frozen a short while before he died.

    its not the top 16 its technically the world champion the tournament champion and 14 highest ranked who are neither

    You can see the "Shot Of The Championship" for each year from 2006 to 2011 on YouTube, but they weren't done as viewers' competitions. There's also the 2001 set which was a competition.

    There isn't a dress code, but one would hope the audience possesses a sense of occasion and dresses appropriately. It's a historic evening, not just any old snooker match.
